Your role will include:
- Supporting individuals as they transition from hospital to home, helping them regain independence.
- Visiting service users in their own homes to provide care as outlined in their personalized care plans.
- Assisting with personal care, administering medication, monitoring vital signs, exercise, and domestic tasks.
- Liaising with medical professionals such as district nurses and pharmacies.
The goal? To help our service users rehabilitate, maximize their potential, and lead more independent lives.
Working Hours:
Shifts are between 7am-9:30pm.
We offer pay rates of £13.50 – £15.50 per hour subject to experience and suitability for a either a junior or senior role.

- Flexible contracts up to 40 hours per week.
- Paid mileage at 35p per mile.
