About the Role
E2E Homecare Ltd is seeking an experienced and dedicated Care Coordinator to join our team in January 2025. The successful candidate will play a pivotal role in ensuring the smooth operation of our domiciliary care services by managing care schedules, liaising with clients and carers, and maintaining high standards of service delivery. This position requires a motivated individual with exceptional organizational and communication skills. A valid driver’s license and access to a car are essential for this role.
Main Duties
Care Coordination:
- Develop and manage care schedules to ensure the efficient delivery of services to clients.
- Match clients with suitable carers based on their needs and preferences.
- Coordinate with carers to manage shift changes, sickness, and holiday cover.
Client Liaison:
- Act as the first point of contact for clients and their families to address concerns and provide updates.
- Conduct client assessments and reviews to ensure care plans remain relevant and effective.
Team Support:
- Supervise and support care staff, ensuring they adhere to company policies and procedures.
- Conduct regular spot checks and appraisals to monitor staff performance.
- Organize and deliver staff training sessions to maintain a skilled and competent workforce.
Administrative Duties:
- Maintain accurate and up-to-date records of client care plans, staff schedules, and other documentation.
- Ensure compliance with CQC standards and other regulatory requirements.
- Prepare reports and provide updates to senior management as required.
Qualifications
- Proven experience in a similar role within the domiciliary care sector.
- Strong knowledge of care planning and scheduling systems.
- Excellent communication and interpersonal skills.
- Strong organizational skills with the ability to multitask and work under pressure.
- Proficiency in IT systems and software relevant to the care industry.
- A full UK driver’s license and access to a reliable car (essential).
- Flexibility to work additional hours when required, including participating in the on-call rota.
