Position: Care Coordinator
Salary: £13.50 per hour

Key Responsibilities:
Care Coordination & Leadership:
- Create, manage, and maintain efficient staff rotas for all clients, ensuring all care visits are covered seamlessly.
- Act as the main point of contact for our team of care assistants, providing them with daily support, guidance, and leadership.
- Assist with recruiting, inducting, and mentoring new care staff, fostering a supportive and skilled team environment.
- Liaise confidently with health professionals (e.g., District Nurses, GPs) and family members to ensure a joined-up approach to care.
Hands-On Community Care:
- Conduct initial assessments and onboarding for new residents/clients, ensuring a warm welcome and a smooth start to their care journey.
- Provide hands-on personal care and support to clients in the community to cover visits, build relationships, and lead by example.
- Perform regular spot checks in clients’ homes to monitor the quality of care provided.
On-Call & Operational Duty:
- Participate in the on-call rota to provide urgent support and advice to care staff and clients outside of office hours.
- Accurately maintain and update client care records, ensuring compliance with CQC standards.
Essential Requirements:
- Must have a recognised Level 3 Diploma in Health and Social Care (or higher).
- A minimum of 2 years of experience in a home care setting within the UK.
- Proven experience in providing hands-on personal care is essential.
- Full UK driving licence and access to your own vehicle.
- Exceptional organisational and communication skills.
