Key Accountabilities:
- Support and empower individuals to develop or improve their independent living skills as outlined in person-centred plans, which may include assistance with domestic or social skills and personal care.
- Collaborate closely with Team Leaders and managers to prepare and implement individual and group support programs.
- Work with other Support Workers and Team Leaders to promote a safe working and living environment, which may involve resolving conflicts or challenging behaviors using appropriate techniques and language.
- Ensure that medication is stored and administered in accordance with Praxis Care’s Medication Policy, as necessary.
- Be familiar with all administrative requirements of the service, ensuring relevant information is accurately recorded and logged as per reporting guidelines.
- Stay informed about all policies and procedures in place within the service (including Health & Safety) and participate in related training sessions.
Requirements of the Role:
- Participate in the company appraisal and staff review process.
- Adhere to and model the company values, behaviors, and competencies at all times.
- Carry out all duties in accordance with Praxis expectations, regulations, and policies.
- Undertake additional duties and responsibilities as required.
- Work necessary hours to meet the needs of the service.
