About the Role
The Moor to Sea Community Health and Social Care Team are seeking a part time permanent Social Worker to join their busy, friendly team.
The role focusses on working with older people with some work with adults of working age as required.
As a Social Worker you will be required to undertake Section 42 safeguarding enquiries; lead on Mental Capacity Assessments and Best Interest processes including applications for community deprivations of liberties. You will need to be able to demonstrate a working knowledge of the Care Act and able to work in a strengths based way to support individuals to reach their eligible outcomes and goals as well as strong risk assessment and risk management skills. You will be working alongside Social Care Assessors and occupational therapists and our health and voluntary sector colleagues following a multi-agency approach.
Moor to Sea covers a large rural area covering the south of Dartmoor down to Dartmouth and into Torbay so you will need to be able to drive and have access to suitable transport.
The team are based at Totnes Hospital however in line with DCC agile working policy there will be opportunity to work from home or other sites.
You will have access to regular supervision and support including group supervision where you will have space to reflect upon key practice issues including theory in practice and research as well as a focus on wellbeing. You will have access to a robust induction.

If successful in being shortlisted for interview, your experience, skills and knowledge, will be assessed against the PCF domains and if found appointable, this will determine your appointment to either the Social Worker (Grade F) or Experienced Social Worker level (Grade G).
With on-going learning and support from your line manager, those appointed as a Social Worker will be able to evidence incremental learning and submit an application in due course for progression to the Experienced Social Worker (Grade G) via a portfolio of supporting evidence against the PCF framework.
