About the Role
An opportunity has arisen to appoint a support worker within CMHRES in Greenwich. This is a unique and exciting opportunity for support workers who may have previously worked in community settings who have an understanding of rehabilitation, enablement and recovery models of care.
To have understanding of relevant and significant Mental health needs/conditions, as well as an understanding or physical health conditions to support service users to have insight and develop independence around their conditions.
To develop and demonstrate good verbal and non-verbal communication skills, adapting to the needs of the service users.
To undertake supervision with line manager and peer supervision if appropriate. Adhere to all Oxleas NHS Foundation Trust, policies, procedures and guidelines.
To work as part of a team delivering high quality, person centred and evidence-based care and support to service users with a mental Health diagnosis focusing on the rehabilitation needs of service whom require intensive support to promote social inclusion and facilitate greater independence. This service aims to enable Service Users to develop the right skills to improve their quality of life through independent living and effective management of their mental health difficulties.
The main aim of the Team is to focus on rehabilitation, engagement, relapse prevention and enablement to optimise the service users’ ability to live independently and enjoy a good quality of life. The post holders will work in conjunction with Care Coordinator’s as an active member of the multi-professional team with a defined caseload of service users who have complex mental health and rehabilitation needs. An ability to work collaboratively with other teams and agencies, good communication and organisational skills will be essential.
Main Duties
- To be part of the team assessing and monitoring physical health, mental health, and needs under the guidance of experienced and qualified staff.
- To plan, organise and deliver person-centred and meaningful activities for service users according to their skills, needs and personal choice, encouraging and facilitating their involvement at all times.
- To keep up to date and clear records for all service users as per Trust and local guidance.
- Use creative, psychosocial and daily living skills to engage Service Users, in therapeutic activities to promote independence, well being and social inclusion.
- To take part in running groups for small groups of service users supported by the team.
- Escort/accompany Service Users to hospital, doctor, dentist or any other relevant appointments with a view to enabling the Service User to become independent in attending such appointments.
- To visit Clients in their own homes, supported accommodation placements and in the community and support them to meet their goals and build independent living skills.
- To facilitate open communication with service users, their families and friends, involving and including them in all aspects of service delivery.
- To adhere to Trust policies and procedures such as Safeguarding Adults and children and Health and Safety policies.
Qualifications
Work-related Personal Requirements (Essential)
- Having a basic understanding of the demands of working with people with Mental health conditions to meet the needs of the client group
- Awareness of Safeguarding Adult Protection issues and the importance of reporting abuse
Qualifications And Training (Essential)
- NVQ Level 3 Health/ Social Care qualification or appropriate equivalent
- Able to work as part of a team and demonstrate a flexible approach
Desirable
- Basic knowledge of health and safety issues
Experience (Essential)
- Experience of working with people with Mental Health conditions
